Coverage for nutrition counseling varies from plan to plan. We are contracted with most major insurance companies and recommend you verify your benefits if you would like to ensure visits will be covered. A physician referral is required for all clients, even those with PPO plans. Use this form to obtain a referral.  When speaking with your insurance company, provide procedure codes 97802 and 97803 and the applicable diagnosis codes from the referral to verify your benefits with your insurance. If we are not contracted with your insurance carrier, you may have out of network benefits. If not, you have the option to pay out of pocket.

Insurance and out-of-pocket payments are accepted. We currently accept Medicare, Anthem Blue Cross, Blue Shield, Aetna, SeaView IPA, Valley Care/Valley Care IPA, United Health Care/UMR, Health Net and First Health. If you have a different insurance carrier, you may have out of network benefits.
